I acknowledge that these are uncertain times. The risks of COVID-19 are not well understood and there is controversy among the experts on how the virus can spread and difficultly in scientifically determining whether anyone has the virus at any moment in time. In consideration for Civello Salon & Spa providing personal services to me, by signing below I agree to accept all responsibility for the risk that I may contract COVID-19. I acknowledge that while Civello Salon & Spa is taking my safety and that of its staff very seriously by employing new safety and sanitation initiatives, I understand and accept that Civello Salon & Spa cannot guarantee that any of these measures will completely protect me from contracting COVID-19. I agree that if I take any steps to make a claim for damages against Civello Salon & Spa, its agents, employees, affiliates, directors, officers or any other parties arising out of my receipt of personal services during my visit to Civello Salon & Spa’s facilities, I shall be obligated to pay all legal fees and costs incurred as a result of such claim. I acknowledge that I can go elsewhere to have personal services, and I acknowledge that Civello Salon & Spa is not the only salon/spa where I can have personal services. By signing this Guest Liability Release, I acknowledge that I am free to go to other salons and/or spas that may not require my agreement to accept responsibility for contracting COVID-19 and I freely and voluntarily choose to have personal services at Civello Salon & Spa.
